Proactively identifies, prevents, and/or resolves drug related problems through participation in medical team rounding. Designs, recommends and monitors patient-specific pharmacotherapaeutic regimens in area of expertise in concert with the medical team. Understands boundaries and requests assistance appropriately. Formulates responses to drug information based on analysis of the literature. During order verification assures appropriateness of medication orders and accuracy of the patient's profile. Chooses daily activities to reflect a priority on delivery of pharmaceutical care. Work with peers in prioritizing the workload and organizing the workflow in assigned areas, taking into account available resources. Serve as preceptor to pharmacy residents in managing order transcription/dispensing and Pharmaceutical Care activities concurrently. Design medication use education for patients and caregivers that effectively meets their needs. Use effective patient education techniques to provide counseling to patients and caregivers including information on drug therapy, adverse effects, compliance, appropriate use, handling and drug administration. Use effective strategies for communicating with patients who are non-English speakers or who are impaired (e.g., blind, deaf, cognitively impaired, illiterate). Maintain Clinical Pharmacist I skill set. Collect and organize all patient-specific information (medication work-up) needed to assure rational, safe therapy for patients on assigned areas. This shall include prevention, detection and resolution of medication-related problems. Exercise skill as a team member in the management of medical emergencies by attending codes as assigned. Design pharmacotherapeutic regimens that meet the goals established for patients; integrate patient-specific, disease and drug information, ethical issues, and quality-of-life issues; consider pharmacoeconomic principles. Design monitoring plans for patients¿ pharmacotherapeutic regimens patient-specific pharmacotherapeutic goals. Recommend pharmacotherapeutic regimens and corresponding monitoring plans to prescribers and patients according to departmental procedures and in a way that is systematic, logical and secures consensus form the prescriber and patient. Modify pharmacotherapeutic plans as necessary based on evaluation of monitoring data. Use the established departmental procedure to systematically communicate pertinent pharmaco-therapeutic information to and from the acute and ambulatory patient-care settings. Attends educational in-services as appropriate. Meets departmental expectations for required programs. Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments. Identify the requester's actual drug information need. Accurately evaluate the usefulness of biomedical literature gathered. Formulate responses to drug information requests based on analysis of the literature that meet the requester's drug information need. Provide appropriate responses to drug information questions that require the pharmacist to draw upon his or her knowledge base because they arise during patient care rounds or other spontaneous situations where reference tools are not available. Consistently follow up on drug information recommendations in order to assess their effectiveness. Exercise skill as a team member in the management of medical emergencies by attending codes as assigned.
MINIMUM QUALIFICATIONS: Graduate of an accredited school of pharmacy; licensure or eligibility for licensure in the state of Georgia. ASHP- accredited PGY2 residency OR ASHP-accredited PGY-1 residency; OR No PGY1 training or lack of training with one year of Emory experience. BLS within six (6) months of hire and ACLS within (1) year of hire, if directly applicable to practice specialty. BPS certification is preferred.

Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock , Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation , Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
- This position will work with pharmacy informatics and oncology specialists in the implementation and maintenance of an oncology Computer Physician Order Entry (CPOE) system. Primary CPOE responsibilities will include the building of oncology ordersets into the system database along with continuous updating and monitoring. Primary clinical responsibilities will include working in the Oncology Infusion Pharmacy assessing chemotherapy treatment regimens, verifying orders, counseling patients and providing education to other healthcare team members. - Minimum of one year oncology experience highly preferred. Previous informatics experience a plus and may be considered in lieu of oncology experience. - 78000BR
