Develops and maintains nursing practice standards of care for a specific transplant population.
Serves as a liaison and provides consultation and education services to support standards of care.
Identifies and resolves clinical issues relating to the transplant patient.
Coordinates the collection of clinical data and patient samples for research studies.
Collaborates with staff, the patient, and his/her family concerning patient evaluation and discharge planning.
Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care.
Coordinates communications with the outpatient transplant population such as transplant clinic visits, community referrals, telephone consultations, arranging for long-range beepers, outpatient treatments, and subsequent hospital admissions.
Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community.
Participates in daily patient rounds with transplant team.
Participates in collaborative, interdisciplinary research activities related to transplant patients.
Performs related responsibilities as required.
A bachelor's degree in nursing or ADN with four years of recent, clinical, medical-surgical nursing experience in the specialty of transplantation and/or outpatient care coordination, with a minimum of two years’ experience in the care of the transplant population preferred.
All nurses employed in roles requiring an unencumbered nursing license must have a valid, active license or temporary permit approved by the Georgia Licensing Board.
BLS certification required.
PHYSICAL REQUIREMENTS (Medium):
20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.

Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock, Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation, Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
