Description

JOB DESCRIPTION:

  • Functions as a preceptor, mentor and coach in guiding and directing the clinical practice of social workers as well as newly hired staff.

  • Regarded as the clinical expert and role model in managing care for special patient population and maintaining collaborative relationships with multidisciplinary colleagues.

  • Demonstrates proficiency in teaching precepting skills for new staff.

  • Serves as clinical instructor for new employees.

  • Identifies education needs/gaps for staff and is responsible for providing required training.

  • Utilizes case management training modules and other clinical tools to enhance best practice.

  • Educate, train and coach staff toward standard work and service excellence.

  • Liaison with local schools of social work for purpose of: networking and developing a student program.

  • Performs quality reviews of staff documentation to ensure compliance with regulatory standards.

  • Demonstrates commitment to own professional development through participation in preceptor meetings and skill development/credentialing/yearly refresher courses/CEUs.

  • Role models and educates others and stays abreast of latest evidence-based practice.

  • Become familiar with Utilization Review Processes.

MINIMUM QUALIFICATIONS:

  • Master's degree required.

  • 3-5 years of acute care experience.

  • License in Social Work required.

  • Certification in case management (ACM or CCM) preferred.

PHYSICAL REQUIREMENTS:

(Medium Max 25lbs) up to 25 lbs, 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 25 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.

ENVIRONMENTAL FACTORS:

Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock, Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation, Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
